WHO IS THIS FORM FOR?
- Any international (Open, Searching or Following) who wants to get connected to a local Christian(s) in their next location (for Mints, include genuinely "Curious" students as well)

INSTRUCTIONS:
This is a STAFF ONLY FORM - not intended for student or alumni use.  While the form may seem long, most of the questions are multiple choice and should not take long to fill out.  The further in advance you fill out a form, the better ability ISM will have to make a successful and secure connection.

BEFORE YOU START you will need:
- Permission and expressed interest from the international student for you to fill out the form on their behalf.  
- Expected graduation/departure date
- City/Country to which they are heading
