IYC 2024 Summer Taiwan SDG Volunteer Service Trip Registration Form (FOR IYC-US only)
Date: August 8, 2024 - August 15, 2024

TENTATIVE ITENERARY
8/7 - Departure from the US, arriving 8/8 morning
8/8 - Group first rehearsal & welcome dinner
8/9 - 8/12 - Two music performances, multiple volunteer and service projects
8/12 - Evening - Farewell Dinner
8/13 - 8/14 - Tour of northern Taiwan (private bus provided)
8/15 - Return to the US

HOTEL STAY
We will be staying at ILLUME TAIPEI for the entire trip. Please book your hotel stay as soon as possible.

COST
~$580.  The cost includes tickets to the National Palace Museum and the Taipei Astronomical Museum, two days Northern Taiwan tour (bus, tour guide, and tips), two IYC T-shirts and a sling bag; does not including hotel stay or air fare.  Lunch is covered for certain days.  Welcome and Farewell dinner is covered). $200 deposit per person due upon registration (VENMO - Henry Tseng @Yite-Tseng OR Zelle - YI-TE TSENG @ 908-922-6888)

VOLUNTEER HOURS
Participating members will receive 100 volunteer hours credit towards PVSA (The President's Volunteer Service Award)

*All minors need to travel with an adult
**Need to register before March 10, 2024 to be included in the music performance

THE PLEDGE
Welcome to the application for the upcoming Impact Youth Club summer Taiwan volunteering service trip.  We look forward to traveling with you!
This trip we organized is group-oriented.  In consideration of fellow travelers, those selected to participate are required to take the following pledge:

1.  I will complete the required orientation before the trip
2.  I will be on time for all activities
3.  I will bring my best to the trip, including my enthusiasm, patience, a listening ear, and an open heart
4.  I will recognize as my equal and show respect for each member of my travel during the trip
5.  Should I take ill, become injured, overly anxious, frustrated or experience any other issues that is interfering with  my experience, I will seek the help of a Trip Leader to quickly resolve the issue
6.  I will follow safety guidelines that will be presented during my Orientation
7.  I will abide by any vaccination policy pertinent to re-enter my home country , and understand that I am solely responsible for completing paperwork or presenting proof required, if any, to re-enter
8.  GROUP HEALTH - If I have cold or flu symptoms during the trip, including running nose, fever, headache, cough, or other symptoms, I agree to mask, wash my hands frequently, and take related precautions to assist in ensuring group health
