Permohonan Bekerja Di Luar Waktu Pejabat di Makmal INBIOSIS / Application to Work Outside Office Hours at INBIOSIS Laboratory
- Sila lengkapkan borang ini dengan maklumat yang betul dan terperinci bagi memudahkan permohonan diteliti serta diluluskan oleh Penyelia dan PIC Makmal. 
- Untuk melancarkan proses permohonan, sila memohon awal sebelum pukul 3PM pada hari ingin bekerja lebih masa. 
- Permohonan bertempoh diberikan kepada pelajar sekiranya kerja yang akan dijalankan dan peralatan yang akan digunakan adalah sama sepanjang tempoh (1 minggu) permohonan.
- Perubahan kepada kerja dan peralatan perlu menggunakan borang permohonan yang baru.
- Sistem permohonan berdasarkan hari dalam minggu yang sama turut dibenarkan untuk membantu dalam perancangan kerja penyelidikan pelajar.
- Kunci makmal perlu dipulangkan kembali kepada PIC Makmal pada hari yang tiada permohonan untuk bekerja di luar waktu pejabat.

- To speed up the application process, please apply early before 3PM on the day you wish to work overtime.
- Term applications are given to students if the work to be carried out and the equipment to be used are the same throughout the period (1 week) of the application. 
- Changes to work and equipment need to use a new application form. 
- Day based within the same week application is also allowed to help in the planning of student research work. 
- The laboratory key must be returned to the Lab PIC on days when there is no request to work outside of office hours.

1) Tidak semestinya daripada makmal yang sama, tetapi perlu memeriksa pemohon setiap 30 minit untuk memastikan pemohon berada dalam keadaan yang selamat.

1) Not necessarily from the same lab, but need to check the applicant every 30 minutes to make sure the applicant is safe.
