1. 750 บาท (ค่าบริการล่วงหน้า 550+ค่าแท่นติดตั้งอุปกรณ์ 200) (***โดยโอนเข้าบัญชี 5652450847 - สำนักนวัตกรรมดิจิทัลและระบบอัจฉริยะ2 ธ.ไทยพาณิชย์ และส่งสลิปมาที่ psufiber@psu.ac.th ) / ฿750( Advance Service Fee ฿550+Installation Equipment Fee ฿200) must pay to SCB Account 5652450847สำนักนวัตกรรมดิจิทัลและระบบอัจฉริยะ2 and email your payment slip to psufiber@psu.ac.th

2. ค่าบริการรายเดือน เดือนละ 550 บาท ยินยอมให้หักจากบัญชีเงินเดือนของข้าพเจ้า ตาม รหัสเงินเดือน ที่ระบุ / I agree to pay from my PSU Salary Code Number provided

3. ค่าแท่นติดตั้งอุปกรณ์ 200 บาท ชำระเพียงครั้งเดียว และไม่มีการคืนเงินนี้ให้ผู้ใช้บริการ / Installation Equipment Fee ฿200 is only one time payment and this will be no refund.

4. ข้าพเจ้ายืนยันที่จะใช้บริการนี้ต่อเนื่องเป็นระยะเวลา 12 เดือน / I agree to use service for 12 consecutive months.

5. เมื่อ สำนักนวัตกรรมดิจิทัลและระบบอัจฉริยะ (DiiS) ได้รับอีเมลสลิป 750 บาท (ค่าบริการล่วงหน้า 550+ค่าแท่นติดตั้งอุปกรณ์ 200) แล้ว DiiS จะติดต่อกลับไปยังผู้ขอใช้เพื่อนัดวันติดตั้ง / When the Office of Digital Innovation and Intelligent Systems (DiiS) received your email with the payment slip of ฿750( Advance Service Fee ฿550+Installation Equipment Fee ฿200), you will be contacted for the installation appointment.

6. ค่าบริการล่วงหน้า 550 บาทนี้ จะใช้เป็นค่าบริการในเดือนสุดท้ายของท่าน โดยผู้ใช้บริการจะยกเลิกได้หลังจากใช้บริการไปแล้วครบ 12 เดือน และต้องแจ้งยกเลิกบริการล่วงหน้า 1 เดือน / The Advance Service Fee ฿550 will be used to pay your last month of service. Service can be cancelled after 12 months only and you must notify DiiS a month in advance.
