DIGITAL CITIZENSHIP INITIATIVE NEEDS ASSESSMENT - PARENTS & STUDENTS
OVERVIEW
Technology has increasingly become an aspect of our daily lives and has much positive potential.  It must be handled responsibly, though, so it is imperative that our students learn how to properly utilize technology within their community and beyond.  For this reason, the Pacific East District will be launching an initiative during SY19-20 to promote Digital Citizenship within its schools to partner with parents and guardians in ensuring students are equipped to interact with technology in a safe and appropriate manner.  Schools in the district will be providing, at minimum, one lesson per semester for each student in their school, upon which further digital citizenship activities can be built.  

PURPOSE
In order to provide lessons over the most appropriate and relevant topics, we are seeking feedback from teachers, parents, and students (6-12 only).  This needs assessment is intended to help identify the digital citizenship topics needed most for your student(s).  Schools will provide, at a minimum, lessons on the top two topics identified by their school's stakeholders, based data provided by this needs assessment.  Furthermore, schools will provide resources throughout SY19-20 to parents in order to further reinforce these topics at home.  

THE NEEDS ASSESSMENT
This needs assessment is anonymous and only requests the respondent's or respondent's child's school and grade band (K-2, 3-5, 6-8, and 9-12) so that topics can be determined for each school and each grade band.  Summarized data for each school for the TOPICS section will be made available publicly by your school after the collection period closes.  Information provided in the ADDITIONAL INFORMATION section will be reviewed by a Digital Citizenship team at each school in order to determine and develop the most appropriate and relevant activities.

Any questions that you might have regarding the Digital Citizenship Initiative and/or this needs assessment can be directed to your school's administrator.

PARENTS: If you have multiple children in different grade bands (i.e. K-2, 3-5, 6-8, 9-12), please consider submitting a separate form for each child.  (If you have multiple children in the same grade band, only one form is necessary for all applicable children.)
