Thank you for registering for the Closet of Hope. Providing this information allows us to verify that you and your family qualify to use the Closet of Hope under our guidelines. Also by filling out this form you agree to use this resource only for the family members who you register. This is a free community resource, and we ask that you respect it and only take what your children need. Under no circumstances can you sell any item you receive from our ministry. Failure to not follow these guidelines will result in a loss of the ability to use this ministry.

IMPORTANT: All information you provide is confidential and will be used only for Closet of Hope (a ministry of Together Church) qualification and communication. To simplify your initial visit you may fill out this form in advance and bring the required documents to the Closet of Hope on the day of your visit.
