Form for THF care boxes for trans people!!
Welcome! This is a form for people who wish to receive a care box from Trans Hope Flourishing. Trans Hope Flourishing aims to support young trans people aged 14 to 25 to flourish. We are giving away free care boxes to support well-being for young trans people. Boxes will include a range of useful information on helplines, dealing with dysphoria and passing, along with free cosmetics, treats and gifts. You can tell us more about yourself  below and the kinds of things you would like to receive and we will do our best to tailor the box to your needs. We will also ask how you would like to receive your box. You can choose to skip any questions you'd prefer not to answer.

This form will take around 5 minutes to fill out. The information you provide will only be used to help us prepare and send your care box and it will not be stored or passed on to any third parties. Trans Hope Flourishing is supported by the Albany, Deptford through the We Move Fund, with the support of BBC Radio 1Xtra & Children in Need.
