Parents or Guardian Information
By signing below, I grant permission (as the parent or legal guardian) for the minor named above to visit the student host’s residence hall at Barnard College. I understand that there may be window stops rather than window guards present in some buildings and that the host is completely responsible for the welfare of this minor. I also understand that if the host or the minor engage in any violation of College policy, they may be required to vacate the premises and I take responsibility for such relocation.

Forms must be submitted at least 2 days in advance of visit (no later than 10am Friday for a visit the same weekend). Requests are not approved until reviewed and a confirmation email is sent to the parent or guardian.
